Diabetes Education

Credentialed Diabetes Educators are specialists in diabetes. They are health professionals who have completed further study to focus their efforts on helping people with diabetes self-manage their diabetes effectively and prevent complications. Educators can provide advice of glucose monitoring and explanations of medication requirements.

A diabetes educator can be the first point of call when you are wanting more information, support and/or motivation in the management of your diabetes and the link between other health professionals. Educators have in-depth knowledge on all aspects of diabetes and can recognise when you need to see other members of your health care team.

 

Type 2 Diabetes Program

The Joint offers a Type 2 Diabetes Program for any patient with Type 2 Diabetes. The Australian Government through Medicare offers patients with Type 2 Diabetes 1 individual assessment with a health professional plus 8 group exercise or education sessions with relevant health professionals. These 8 sessions are fully subsidised by Medicare with NO OUT-OF-POCKET EXPENSE for patients.

**Note: The 1 x assessment and 8 x  group sessions are in addition to the 5 session available through the Chronic Disease Management Plan and are available each calendar year**

Our team of highly experienced Exercise Physiologist, Dietitian and Diabtes Educator will provide a thorougher individual assessment and based on this, will advise on which sessions will be most beneficial for the patient. Following the assessment and advice from health professionals, patients will undergo 8 small group exercise and/or education sessions to optimise their management of their Diabetes.
